Client Profile

Create a Client Profile with just the required information to get a participant started in the system.

  • Login with your credentials.
  • Click the Client menu option.
  • Click the Profiles menu option.
  • Click the Add button on the Client screen
  • Client Type: defaults to Participant and is read-only.
  • Enter First name:  required field, it must be filled in.
  • Enter Last name:  required field, it must be filled in.
  • Click Save.

Collecting Participant Profile Details

  • Client Type: Read-only required field that is auto filled by system.  It defaults to participant or family member depending where the individual is added in the application.
  • Social Security No.: SSN number for the individual. Optional field, uniquely identifies the individual, used to perform duplicate checks if available.
  • First Name: First name of the participant.
  • Middle Name: Middle name of the participant.
  • Last Name: Last name of the participant.
  • Email Address: Email id of the participant.  It is unique.
  • Gender: Gender of the participant. The values in drop-down are not available for editing at this moment.  See LookList Values for details.
  • Date of Birth: is for capturing the participant's date of birth.
  • Mobile Number: an optional field that is the phone number of the participant.
  • Alternate Number: an optional field for an additional phone number of the participant.
