Referral Establishments

• Login with your Access.
• Click the Referrals menu option.
• It will expand to display additional options.
• Click the Establishments menu option.
• Click the Add button on the Establishments screen.
• Enter Name: Name of the Organization that accepts Referrals
• Enter Referred Services : Select the services offered by the Organization.  There is a comprehensive list of services to choose from. Make your selections by checking the box. Users can search for services by typing search criteria into the search bar.
• Click Save
  • Establishment is created successfully and the Address details portion displays. 

Collection Establishment Details

Most of the details entered into the Establishment profile are used on the Referral Page.

• Contact Name: Name of the person to be contacted at the Organization.
• Phone Number: Phone number to contact
• Email: Email for communication 
• Website: Link to the webpage of the organization.
• Facebook: Link to facebook page of the organization.
• Address : Address details will be used in the Referral Search.  An organization without an address will not display in the search results.

Establishment Address

Each Establishment can have an Address.  Click to add Address Details.

• Click the Add button on the Establishment Address Section.
• Add Address  screen displays.
• Address Type:  Users can choose the type of address.  Primary, Alternate or Anticipated Address.
• Address Line 1: Fill the Street address.
• Address Line 2: Fill Apartment or House Number.
• Zip: Fill the 5 digit US Zip Code
• City: Fill the City name
• State: Select the State name
• Click Save.
